  • Failure to guarantee that each resident is not subjected to physical restraints unless it is deemed necessary for medical purposes – citation #F221 issued on April 7, 2017.
  • The investigators stated that residents who had a decrease in functional mobility and potential seclusion were affected by this failure. The nursing home failed to develop a Care Plan for two residents and obtain the doctor’s orders, as determined by the investigators.

The state surveyors observed both residents. One resident mentioned that he was in his bed with fully raised side rails, while the other resident stated that she was sitting on her wheelchair with a lap belt. The resident in bed expressed that he did not require the side rails, whereas the resident in the wheelchair mentioned that she was unable to remove the lap belt on her own.

In compliance with legal regulations, the survey team conducted an interview with a Licensed Vocational Nurse who verified that the lap belt of the other resident did not have any directives from a physician or Care Plan, and the bedside rails were elevated for [one resident].

Failure to Develop Programs That Examine, Regulate and Prevent Infections from Spreading – citation #F441 dated September 7, 2017.

The “facility” failed to maintain infection control procedures and policies when the CNA went between two isolation rooms without washing his hands or using personal protective equipment (PPE) to protect the residents.

The investigators determined that this failure in the facility observed cross-contamination of harmful bacteria, which exposed an already compromised population to potential harm. Surveyors observed that the CNA walked into an isolation room without wearing any personal protective equipment and had contact with a highly contagious resident. This repeated contact with the resident, without wearing any protective gear, increased the risk of harmful bacteria spreading.

The registered nurse verified this malfunction by visually inspecting it, after which the certified nursing assistant exited the room of that particular resident “without performing hand hygiene and proceeded directly into the second isolation room without donning personal protective equipment to provide care for [the second resident].”
